PfizerFlex™ Patient Support Program Terms and Conditions – Australia and New Zealand

By opting into a PfizerFlex Program, you accept and agree to these Terms and Conditions, together with Pfizer's Privacy Policy and Website Terms of Use.

For the purposes of these Terms and Conditions, “Pfizer” means Pfizer Australia Pty Ltd, Pfizer New Zealand Limited and, where applicable, any applicable affiliates or nominated third-party service providers engaged by Pfizer to deliver the programs.

  1. You understand and acknowledge that participating in a PfizerFlex™ Patient Support Program is optional and designed to complement and support the prescribing instructions of your healthcare professional. You must contact your healthcare Professional for any queries regarding your treatment or condition.
  2. You confirm you are a resident of Australia or New Zealand and have been prescribed and dispensed the Pfizer product for the specified indication associated with the PfizerFlex™ Patient Support Program you are participating in. These criteria will be verified on an ongoing basis at the time of logging in to PfizerFlex and Pfizer reserves the right to discontinue your access to a Program if you no longer meet the criteria.
  3. You must be 18 years or older to enrol or use a PfizerFlex™ Patient Support Program. Participation in a PfizerFlex™ Patient Support Program by anyone under the age of 18 must be administered by a parent/guardian.
  4. You may opt out of a PfizerFlex™ Patient Support Program by calling (AU) 1800 793 539 or (NZ) 0800 793 539 or by emailing [email protected] (patients on Pfizer's endocrine medicines should instead email [email protected] and Enbrel medicine should instead email [email protected]). If you opt out of a PfizerFlex™ Patient Support Program, personal information collected may be held for purposes permitted by law. All personal information is handled in accordance with Pfizer’s Privacy Policy.
  5. Pfizer reserves the right to change and/or terminate a PfizerFlex™ Patient Support Program with reasonable notice at any time.
  6. The services provided through a PfizerFlex™ Patient Support Program will vary depending on the patient's prescribed treatment and individual needs. By opting into a PfizerFlex™ Patient Support Program, you consent to the relevant Program services to be delivered by Pfizer or nominated third-party service providers. Services may include:
    1. Patient Portals for online disease and treatment information, tools and resources;
    2. Nurse Services including product training, telehealth and in-person consultations;
    3. Health Coaching Services;
    4. Patient Support Merchandise such as care packs, travel kits and sharps containers associated with the Pfizer medication;
    5. Appointment and treatment reminders; and
    6. Phone and email support.
  7. You may enrol in a PfizerFlex™ Patient Support Program either by:
    1. self enrolment online at https://www.pfizerflex.com.au in Australia or https://www.pfizerflex.co.nz in New Zealand;
    2. phoning 1800 793 539 in Australia or 0800 793 539 in New Zealand; or
    3. referral by your healthcare professional.
